Required Documents for New Player Registration

All new players MUST submit the required documents in one PDF file to registrar@wvmha.ca PRIOR to registering. Once the required documents have been received and reviewed by the registrar, you will receive an email with your player’s Hockey Canada ID# and the link to register.

If your player is transferring from another minor hockey association, please contact the registrar directly at registrar@wvmha.ca.

Proof of Residency in West Vancouver (2 different pieces) eg utility bill, bank statement, BC medical statement, property tax statement. Note: mobile phone bill and driver’s license not accepted.
Passport

Copy of Visa or PR Card or Canadian Passport of player

Copy of documentation for parent showing either Canadian citizenship, Landed Immigrant status, Permanent residence (PR) status, work permit or documentation showing PR has been applied for (work visa)
